Forms can be customized with a simple drag and drop interface called Form Builder. Form Builder is launched from Form Center via the following actions:
Create a New form: click “Create new Form” on the top right to create a new form from scratch
Copy an existing form: click the “copy” icon to create a new form using an existing form as a template
Editing existing form: click “pencil icon” within the Actions column of the form. Only forms with “Draft” status can be edited
NOTE: once a form is published, it can no longer be edited
Form Builder
Form Builder contains 4 tabs to help you design and preview your form:
- Form Information
- Form Editor
- Preview Form
- Sample Data Export
Form Information: When creating a new form, you will be prompted to provide a form title and form description. Form description is for your own reference and will be displayed in the Form Center dashboard table. The form title will also be displayed for users when selecting forms to add to a notebook page, so be sure that titles are clear and unique.
Form Editor: Use Form editor to build your custom form.
Adding Fields: You can add different field types to your form by clicking on or dragging and dropping the options on the left panel
- Text field: simple short text field
- Text area: long text paragraph field
- Number: records only numeric values
- Radio Group: multiple choice single-select with radio buttons
- Checkbox Group: multiple choice multi-select with checkbox buttons
- Select: multiple choice dropdown list, can be set up as single-select or multi-select
- Date field: record date using regional date format
Editing fields: Drag and drop or use the arrow keys to rearrange fields. Field labels and select options can be customized:
- Label: input a custom field name
- Options: name your options for Radio Group, Checkbox Group, and Select fields. You can also set a default selection for this input field by selecting the radio or checkboxes next to the option name.
- Required Checkbox: By checking this box, users will be required to complete this field when adding this form to a notebook page
- Use the trashcan icon to delete fields or options
Preview Form: use the Preview tab to view how your form entry would be displayed when added as an entry to your notebooks.
Sample Export Data: Preview the data columns for your form’s CSV export. This includes metadata columns that are standard for all form data exports.
